Job Summary:

Provides comprehensive physical therapy services to patients, focusing on restoring and improving physical functions through individualized treatment programs. Collaborates within an interdisciplinary team to deliver quality care aimed at patient recovery and well-being.

Responsibilities:

• 
  Conduct thorough evaluations to assess patient needs and develop personalized treatment plans.
  
  
• 
  Implement evidence-based therapy interventions utilizing appropriate techniques and tools.
  
  
• 
  Monitor patient progress and adjust treatment protocols accordingly to optimize outcomes.
  
  
• 
  Document clinical assessments, treatment plans, and progress notes accurately and within established deadlines.
  
  
• 
  Supervise and collaborate with assigned assistants and support staff.
  
  
• 
  Educate patients and their families on treatment goals, safety, and post-discharge care instructions.
  
  

Qualifications:

• 
  Hold a Doctor of Physical Therapy (DPT) degree obtained on or after January 2016, or an equivalent foreign credential.
  
  
• 
  Possess a valid state license to practice physical therapy.
  
  
• 
  Maintain current CPR or BLS certification from an approved provider.
  
  
• 
  Have at least two years of recent experience in the physical therapy field preferred, with three years preferred.
  
  
• 
  Demonstrate proficiency with clinical documentation, preferably including EPIC experience.
  
  
• 
  Be willing to work day shifts, Monday through Friday, with no on-call required.
