• LA Orange County Specifically Long Beach, Torrance, Rancho Palos Verde
• Panel - 50-90 patients total; daily load depends on acuity. Some patients are seen monthly and some are seen biweekly.
• Days - Monday through Friday 8AM-5PM
• Duration - 3 months
• Patient Population - MediCare
• Experience needed: Geriatric, Hospice, end of life, chronic disease management, time management, ability to work autonomously, adapt to change, flexible
