Social Worker Care Manager I

Job Summary Inpatient Social Worker Care Manager I (SWCM I) facilitates the collaborative interdisciplinary case management process, including assessment, planning, facilitation, care coordination, and evaluation, appropriate to the scope of licensure in a hospital setting. The SWCM I helps patients and families address and resolve the social, financial, and psychological problems related to their health condition, advocates for services to meet comprehensive needs, and promotes quality cost‑effective outcomes. The role involves clinical social work and complex discharge planning for patients with psychosocial needs, assistance with eligibility determination for social programs, crisis intervention and mental health assessment, and participation in multidisciplinary discharge planning and team meetings. The SWCM I also participates in quality improvement activities and exemplifies professionalism, promoting a customer‑friendly environment by utilizing ServiceFIRST behaviors. Minimum Qualifications Degree: Graduation from an accredited school of Social Work with a Master’s degree in Social Work. Licensure/Certification:

• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with a current licensure in the State of Texas.
• Certified Case Manager (CCM) or Accredited Case Manager (ACM) (both preferred).
• Basic Life Support.

Work Experience: Prior experience as a social worker preferred. Communication Skills: Above‑average verbal (heavy public contact), exceptional verbal (e.g., public speaking), writing and composition (correspondence/reports). Bilingual Skills: Spanish preferred. Proficiencies: PC, MS Word. Job Attributes: Analytical, medical terminology, mathematics. Work Schedule: Weekends, holidays, flexible, travel, on‑call.
