Licensed Physical Therapist, Per Diem

Apple Rehab is a short-term rehabilitation and skilled nursing facility located in Avon, CT. Position Summary Physical Therapists (PTs) provide medical services to help individuals recover or improve function after injury or illness. PTs evaluate patients, develop and implement treatment plans, and collaborate with other healthcare professionals to maximize quality of life. Position Responsibilities Plan and administer medically prescribed physical therapy treatment for patients with injuries or musculoskeletal, neurological, or orthopedic conditions. Review physician referrals, patient histories, and medical records to determine appropriate therapy. Test and measure patient strength, motor development, sensory perception, functional capacity, and respiratory/circulatory efficiency, and record findings. Prepare written treatment programs based on evaluation data. Administer manual exercises to improve and maintain function. Instruct, motivate, and assist patients in performing non‑manual exercises, ambulatory functional activities, and daily living activities. Use assistive devices such as crutches, canes, and prostheses as needed. Apply physical agents (hydrotherapy, moist packs, ultraviolet/infrared lamps, ultrasound) and adjust treatments for maximum effect. Administer massage and traction to relieve pain. Document treatment, response, and progress in patient charts or electronic records. Educate patients and families on home exercise and self‑management procedures. Collaborate with physicians and other practitioners to refine treatment plans and integrate care. Direct assistants and aides in their work activities. Perform additional duties as assigned within the facility setting. Complete required orientation and training as directed. Follow facility and OSHA safety procedures, GHR Unusual Occurrence Protocol, cultural and religious practices, and HIPAA regulations. Maintain punctuality and reliability for scheduled shifts. Required Qualifications Graduate of an accredited PT program. Current PT license in good standing with the state licensing board. Federal, State, and GHR‑required credentials current and on file. Cognitive skills relevant to the position. Working Conditions Evening and weekend work may be required to accommodate patient schedules. The position is physically demanding: PTs stand for long periods, stoop, kneel, crouch, lift heavy equipment, and assist with patient transfers or lifts. Physical Demands Ability to lift 50 pounds routinely and up to 100 pounds as needed. Ability to stand for extended periods.
