Hospice Certified Nurse Assistant (CNA) /Home Health AideEmployment Type: Full timeShift: Monday - Friday, 8am - 5pm, No Weekends or Holidays!We have opportunities in the following areas:Waterbury RegionMeriden RegionHartford RegionMust be a Certified Nurse Assistant in the state of ConnecticutPosition Purpose:Our hospice home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.What You Will Do:Follows care plan developed by RN.Reports client changes in condition to RN, as described on care plan.Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.Performs range of motion and other simple procedures as an extension of therapy services as assigned.Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record.
