Physical TherapistProvide physical therapy services within the legal scope of practice for the state of practice, following all licensure requirements. Maintain valid licensure and certifications as required by state and federal regulations. Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare, state, and CareM standards. Develop, implement, and update individualized plans of care aligned with physician orders and patient needs. Deliver skilled interventions and maintenance therapy programs as permitted by state defined scope of practice.
