Responsibilities 3. Responsible for handling inbound calls from patients, hospitals and/or medical offices who wish to consult or make an appointment with a physician. 4. Ensures patient personal & geographical data is correct and up to date. 5. Ensures all pertinent information is requested at the time the appointment is scheduled (reason for visit, any x-rays and insurance, etc.) and is documented in the note section with the initials of the staff who made the appointment. 6. Rescheduling of patients due to surgery schedules, office hour changes, etc. 7. Provides back up assistance to Care Coordinators as needed. 6. Inform Administration of any change in a resident’s condition. 8. Encourage socialization among residents. 9. Encourage and/or remind residents to participate in activities. 10. Promote activities enthusiastically to pique residents' interest. 11. Chaperone activities outside the facility when requested. 12. Monitor health, safety and well-being of the residents. 13. Document information pertaining to changes in residents' functional status, personal needs, appointments, etc. 14. Assist in keeping the environment safe for residents. 15. Provide one-on-one support/visitation to assigned residents on a weekly basis or as requested by REPORT TO or as specified in the assistance plan or daily communication log. 16. Report concerns about residents to Administration. 17. Respond to resident emergencies; provide first-aid assistance and arrange for appropriate medical attention/follow-up. 18. Arrange for transportation services for transfers. 19. Identify supplies needed to perform tasks and report needs to REPORT TO. 19.
