Bridge Home Healthcare is a CMS-certified home health agency dedicated to providing high-quality, compassionate care to individuals in their homes. We are seeking an experienced HOME HEALTH AIDES to provide patient care, and ensure compliance with home health regulations.

Job Summary
We are seeking a compassionate and dedicated Home Health Aide (HHA) to join our healthcare team. The ideal candidate will provide essential support to patients in their homes, assisting with daily living activities, monitoring patient health, and ensuring a safe and comfortable environment. Under the supervision of a Registered Nurse (RN), the HHA will offer personal care and basic health-related services to improve the quality of life for patients. Flexible hours are available.

Responsibilities

• Assist patients with daily living activities such as bathing, dressing, and grooming.
• Provide compassionate care and support to patients, including those with terminal illnesses.
• Work with individuals with disabilities, ensuring their comfort and safety.
• Prepare meals and assist with cooking as needed.
• Launder clothing and linens for patients to maintain cleanliness.
• Perform catheter care and assist with other personal hygiene needs.
• Measure and record vital signs, including temperature, pulse, and blood pressure.
• Maintain patient confidentiality in accordance with HIPAA regulations.
• Observe patients for any changes in condition or behavior and report findings to nursing staff.
• Ensure the cleanliness of patient areas and assist in maintaining a hygienic environment.
• Complete visit records accurately and in a timely manner per agency policy.
• Perform household tasks essential to patient care as assigned.
• Aid in patient mobility and exercises as assigned.
• Perform simple procedures as an extension of therapy or nursing services, such as range of motion (ROM) exercises.
• Assist with self-administered medications as directed.
• Participate in in-service and continuing education programs as needed (yearly CEU requirements provided by the agency).
• Attend patient care conferences as scheduled.
• Other supplemental duties as assigned.

Qualifications

• Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
• High school diploma preferred.
• HHA, CNA, or STNA certification preferred prior to hire.
• Must have a form of reliable transportation.
• Have a compassionate and patient-centered attitude toward the care of the sick and elderly.
• Ability to follow directions, read, and write.
• Maturity and ability to handle the responsibilities of the role effectively.

Lifting Requirements

• Ability to participate in physical activity.
• Work for extended periods while standing and being physically active.
• Perform heavy lifting.
• Engage in extensive bending, lifting, and standing regularly.
