Home Health RN – Home Health

Shift: 8-hour days only; weekends: not required; on-call: no

Assignment Details:

• Patient Load / Ratios: 1:1; may visit up to 6 patients/day
• Guaranteed Hours: Facility may cancel up to one shift per two-week pay period

Requirements:

• Minimum of 1 year of skilled home health RN experience
• OASIS experience required
• Licensure: OR & WA RN licensure required (pending licenses accepted)
• Required certifications: BCLS
• Key clinical skills: start/maintain IVs, phlebotomy, medication administration (IM/IV/subQ), admission/start-of-care, recertification, resumption of care, discharge, pain assessment/management, infection control

Roles & Responsibilities:

• Provide direct patient care to adults and geriatric patients in a home health setting.
• Administer medications and treatments as prescribed.
• Perform assessments and develop care plans based on patient needs.
• Educate patients and families on disease management and care techniques.
• Ensure compliance with federal and state regulations regarding home health care.
• Document patient care and progress accurately in the required systems.

Compliance / Notes:

• Orientation: 40 hours
• Modules (~16 hours) required pre-start and annually
• Parking: Free
• Scrubs: Business casual preferred; scrubs allowed (no jeans); no specific color required
