In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services./ppEssential Responsibilities/pulliReceive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals/liliAssess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families/liliEducate patients and families on what hospice care includes, how it works, and why it mattersproviding clarity, reassurance, and compassion/liliComplete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met/liliServe as a liaison between referral sources and our hospice care teamensuring seamless handoffs, clear communication, and rapid response to needs/liliMake daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians/liliBuild and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services/li/ulpCompensation may vary within the salary range provided based on several factors including but not limited to a candidates location, experience, education, skills, licensure, certifications and department equity.
