Licensed Practical Nurse

Summary Licensed Practical Nurse responsible for providing prescribed medical treatment and personal care services under direction of an RN or physician to address the medical or psychiatric needs of individuals with developmental disabilities, acquired brain injuries, substance abuse issues, and/or emotional or behavioral challenges across multiple sites. Travels between locations and may require accessibility after hours. Essential Job Functions Provides basic care and medical treatment to individuals receiving services. Completes health assessments and provides input for nursing care plans. Follows up on all physician orders. Monitors and ensures follow through on individuals’ medical/dental appointments. Monitors and records medications in a timely manner, acting as liaison to pharmacy, reviewing medication error forms, administering medication as required, overseeing staff medication administration, and conducting quality assurance checks on health care records. Provides emergency coverage and/or care for individuals receiving services. Maintains confidentiality, respects human rights and practices universal precautions; upholds policy and procedure development related to health care. Partners with external agencies to coordinate services and ensure consistent treatment. Administers, tracks, and maintains logs for staff members’ and individuals’ testing. Provides training to staff members in medication administration, universal precautions, infection control, and to individuals in self‑medication, prescribed treatments, sex education/STD prevention, and smoking cessation. Coordinates household activities such as light housekeeping, meal preparation, and transports individuals to necessary activities and appointments. Travels between multiple locations during work hours, provides support and is accessible to staff at all locations during working hours; may be required to be accessible after hours for consolation. Participates in on‑call system as needed. Performs other related duties and activities as required. Supervisory Responsibilities None Education and Experience Associate’s Degree in Nursing or related field, or graduate of an accredited LPN program. One year experience working with the population served in a nursing capacity. Certificates, Licenses, and Registrations Current state LPN license. Current driver’s license, car registration, and auto insurance if providing transportation. Other certifications required by state licensing regulations such as CPR/FA, HIV/AIDS certification, BBP, and ACT training. Blood Borne Pathogen Category Job requires performing duties that involve potential exposure to blood, body fluids, or tissues. Physical Requirements Heavy work; may require exerting up to 100 pounds of force occasionally, up to 50 pounds frequently, or 20 pounds constantly. Other Requirements Travels between multiple locations.
