Case Manager Nurse - Full Time/Part Time - Home Care

Benefits: Competitive salary Flexible schedule Opportunity for advancement Job Summary We are seeking a dedicated and compassionate Registered Nurse (RN)/Case Manager Nurse to join our growing home care team. This full-time/part time RN position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Northern Virginia communities. Responsibilities Conduct initial client assessments and routine reassessments in client home settings Conduct hospital discharge assessments in client home settings Develop and update individualized care plans Ensure compliance with care standards Qualifications Must hold a current, unrestricted Registered Nurse (RN) license in the Commonwealth of Virginia Must have experience conducting in-home clinical assessments and completing Medicaid DMAS Forms 97, 98, 99, and 100 Ability to work independently Benefits Flexible Scheduling Competitive Compensation Paid mileage for eligible travel Flexible work from home options available. #J-18808-Ljbffr Five Stars Care LLC
