About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience required License: Current nursing license in hand required Certifications: CPR, BLS required Must-Have: Proficiency in HomeCare HomeBase and OASIS Description: Provides coordinated and directed home nursing care according to physicians' orders. Nursing care is based on agency policies and procedures through competent application of the nursing process. Responsible for supervision and evaluation of Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses. Degree of travel includes home visits and periodic office meetings. Must have reliable transportation, a current driver’s license, and appropriate liability insurance. The average number of patients seen per day is 5-8. Expected weekly productivity points are 30, with each visit and activity holding different point values. Types of cases include Start of Care, Wound Care, and follow-up visits. Must be willing to travel up to 75 miles from the assigned area. Mileage will be reimbursed at the IRS standard rate. Must be proficient in electronic medical record systems and have experience within the last 6 months. All documentation must be completed within 24 hours of patient appointments. No guaranteed hours are included with this contract. All clinicians must understand that they will be expected to accept every patient on their schedule regardless of distance. Completion of documentation that impacts billing and patient care is essential. On-call requirements and weekend schedule may vary per location. All travelers must comply with home health standards and practices.
