Position: Occupational Therapist for Home Health

Location: Missoula, MT

Job Summary

The Occupational Therapist is responsible for evaluating and implementing rehabilitation interventions in a home setting. This role involves the management and supervision of patient care alongside educating patients and their families.

Essential Duties and Responsibilities

Evaluate and establish a comprehensive plan of care tailored to meet patients' needs in their home environments.
Coordinate care with other healthcare disciplines and arrange for additional services as needed.
Provide direct patient care based on the established care plan and physician orders.
Utilize knowledge of evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in evaluations and treatment techniques.
Modify treatments as necessary, including adjustments to frequency and duration of care.
Master scoring of the CMS required OASIS and collaborate with the Quality team for necessary adjustments.
Maintain an understanding of insurance requirements in the Home Health field.
Participate in multidisciplinary conferences to create relevant patient care plans.
Communicate effectively with physicians and multidisciplinary team members.
Practice point of care documentation for all direct patient care activities.
Document all skilled services provided to patients, including any adverse events and relevant communications in the medical record.
Engage in Quality Assurance and Performance Improvement initiatives to help the agency meet its objectives.
Manage and meet productivity standards in alignment with agency expectations.
Track and maintain accurate time logs and mileage.
Communicate daily with a scheduler regarding worked hours.
Maximize continuity of care and optimize travel routes when scheduling visits.
Supervise services provided by the Occupational Therapy Assistant and conduct supervisory visits within required timelines.
Oversee the activities of Certified Nursing Assistants (CNA) once their involvement in the care plan concludes.
Participate in care conferences as needed.
Plan admissions, OASIS time points, and discharges throughout the episode of care.
Maintain occupational therapy licensure requirements, including continuing education.
Adhere to ethical standards set by the occupational therapy licensure board and agency policies.
Participate in agency-provided educational opportunities as scheduled.
Perform other duties as assigned by the supervisor.

Minimum Qualifications

BS degree or higher from an accredited Occupational Therapy program.
Current license to practice in the State of Montana.
Valid driver's license and appropriate auto insurance.
Two years of previous experience in Occupational Therapy is preferred.
