Case Manager

Rock County Restorations Sober Living Home provides dedicated support for women on the path to recovery. Restorations is a safe place for women for transitional living and long-term recovery from substance abuse while focusing on restoring lives through the love of Jesus Christ. We aim to provide hope through individualized, supportive, and compassionate care. Role Description This part-time, on-site Case Manager role is located in Janesville, WI. The Case Manager will support individuals in their recovery journey by coordinating care plans, connecting clients to resources, and monitoring progress. Other responsibilities include conducting assessments, maintaining detailed client records, assisting with goal setting, and advocating for clients’ needs while ensuring privacy and compliance with organizational policies. Qualifications Experience in case management, client care coordination, and resource referral services Knowledge of addiction recovery, including 12-step programs and faith-based recovery approaches Strong organizational, record-keeping, and documentation skills Empathetic communication, active listening, and interpersonal skills Ability to work on-site and collaborate effectively as part of a team Familiarity with using computer systems and creating reports Bachelor’s degree in Social Work, Psychology, or a related field is preferred Prior experience working in substance abuse recovery or lived experience is an advantage #J-18808-Ljbffr
