Schedule : Monday - Friday; 8am-4pm w/flexibility as needed

Position Overview

The Assistant Supervisor Case Manager is a hybrid role responsible for carrying a small part-time caseload while providing direct operational oversight to the Columbia County Homeless Intervention Team. This role reports directly to the Program Director and assists with reporting, paperwork, and team leadership.

Position Specifications & Requirements

• Education: Associate’s Degree required ; Bachelor’s Degree in human services or a related field preferred.
• Experience: At least one (1) year of supervisory experience, with a proven track record of advocacy for homeless or vulnerable populations.
• Technical Skills: Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint), Google Workspace, and secure internet practices.
• Communication: Outstanding written and verbal communication skills with strong attention to detail.
• Credentials: Must have a clean, valid New York State Driver’s License (licensed for at least 1 year) and reliable transportation. Ability to pass required pre-employment background checks and fingerprinting.
• Flexibility: Ability to work hours beyond a regular workday, including a rotation of holidays.

Key Responsibilities

• Case Management (Part-Time): Develop and implement comprehensive service plans for households experiencing or transitioning out of homelessness. Engage and assess client needs (employment, medical, behavioral health) and actively connect them with critical community-based resources and protective services.
• Supervision & Leadership: Train, coach, and supervise a small case management team. Manage the referral and intake process, facilitate bi-weekly note reviews, and lead case update meetings.
• Collaboration & Compliance: Facilitate monthly joint case reviews with the Department of Social Services (Prevention and Adult Protective Services). Ensure strict program compliance and up-to-date record keeping across database platforms (AWARDS, client rosters, and tracking spreadsheets).

Duties & Responsibilities include but are not limited to

• Program Supervision: Provide direct oversight of the day-to-day operations of the Homeless Intervention Program, including direct supervision, training, and coaching of the Homeless Case Management Team.
• Referrals & Intakes: Manage and oversee all program referrals, staff assignments, and assist with completing intake processes when needed.
• Coverage & Support: Provide temporary leadership and support to the Galvan Civic Motel Case Management Team in the absence of the Program Director.
• Caseload Management: Carry a part-time caseload of 4–8 families/individuals referred by the Columbia County Department of Social Services, providing direct, strength-based case management.
• Service Planning & Advocacy:
  • Assist clients in developing service plans that address housing instability, secure public benefits (SSI/SSDI, Public/Temporary Assistance), and remove barriers to permanent housing.
  • Act as the primary advocate and liaison for families when communicating with public assistance and other resource providers.
  • Provide transportation for families to medical, social services, and housing appointments when appropriate.
• Meetings & Committees: Participate in the Education and Outreach Committee, Housing Advisory Board Committee, and assist the Program Director in organizing community outreach events.
• Assessments & Documentation:
  • Complete Social Determinants of Health checklists (at baseline and 6 months) and Coordinated Entry Applications.
  • Maintain highly accurate, up-to-date records, including Consumer Face Sheets in AWARDS, the Housing Application Tracking Spreadsheet, and client rosters.
  • Document all client interactions via progress notes, assessment forms, and critical incident reports.
• Crisis Intervention: Intervene in crisis situations and provide mediation and conflict resolution (particularly landlord-tenant disputes).
• Confidentiality: Strictly adhere to all HIPAA requirements and maintain absolute client confidentiality while handling Protected Health Information (PHI).
