We're seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Carease Health - IL, a nationwide preventative healthcare company committed to reducing hospitalizations. As a hybrid role, you'll have the opportunity to work on-site in skilled nursing facilities while also enjoying the flexibility of remote work from home.

About the Role:

As a Nurse Practitioner or Physician Assistant, you'll play a vital role in our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to prioritize proactive measures for enhanced physical and mental health. You'll work closely with our experienced clinical team to identify and address health concerns, providing exceptional care to patients in skilled nursing facilities.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home

• Utilize a patient-centered approach, building strong relationships with facilities and patients

• Collaborate with our dedicated scribe team for efficient documentation

• Stay up-to-date with industry developments and best practices through continuing education and training

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
