Qualifications Valid California PT license At least two years of outpatient experience, including orthopedic Prior experience with Kaiser Permanente preferred SSN and DOB required Facility Recipient of The Joint Commission’s Gold Seal of Approval, Kaiser West Los Angeles Medical Center.
