Outpatient Physical Therapist

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans based on patients' needs, goals, and physician recommendations. The role involves the delegation and direction of all discipline-specific therapy services for designated patients while ensuring the design of functional treatment programs that facilitate effective return to community activities. Additionally, this position serves as an educator for patients, families, students, coworkers, and self, while performing various professional duties associated with patient care and the development of therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Document medical records clearly, accurately, and in a timely manner, including goal writing and charge entry.
• Ensure care is provided under signed, valid physician orders and in compliance with insurance requirements.
• Participate in student programs as assigned, providing positive learning environments and appropriate supervision.
• Create individualized professional development plans, actively pursuing continuing education and professional growth.
• Adhere to discipline-specific state practice acts and maintain licensure in good standing.
• Understand and implement the organization's performance improvement methodology to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for Physical Therapy.
• Experience with Select Medical or a similar agency is preferred but not required.

Schedule

This position operates on a 5x8.0 schedule.

Assignment Duration

This is a 13-week contract position.
