And most importantly, as the only non-profit hospice provider in the region, we never turn anyone away due to an inability to pay.The Pediatric RN is responsible for planning and managing the pediatric nursing care of an assigned caseload of patients primarily in the home care setting. In collaboration with the interdisciplinary team, the RN ensures that patient and family member directed goals are achieved using a holistic, patient-centered approach. The role includes providing education, facilitating communication, and coordinating care across various sites. Flexibility working across patient site locations and care settings are required.Position DetailsJob Location: Home care and hospital settings, predominantly in Duval County and surrounding countiesDays/Hours: Monday - Friday 8:30am - 5:00pm; usually one weekend every six-weeksCompensation: $78,000-$80,000 annual, plus paid driving mileage; full benefits package including 23 days annual PTOJob ResponsibilitiesConduct initial and follow-up assessments to identify patient and family needs.Implement and follow the physician-ordered plan of care, including medication administration, treatments, and support through discharge or end-of-life care.Document all patient care interventions, including medications administered and post-mortem care when applicable.Communicate promptly with management regarding patient issues, concerns, or service-related feedback.Adhere to infection control practices and patient confidentiality protocols.Provide education and advocacy to patients and caregivers, promoting safe and dignified care focused on quality of life.Coordinate patient scheduling, enrollments, and required 60-day nursing visits.Serve as a liaison between the pediatric interdisciplinary team and other care providers.Maintain updated knowledge of pediatric programs and services across the organization.Participate in interdisciplinary group (IDG) meetings and team discussions.Ensure timely and accurate completion of clinical and organizational documentation.Participate in community outreach, in-service training, and program-related presentations as appropriate.Becomes dually employed with Alivia Care, Inc.'s home health agency, Alivia Home Health, and performs home health responsibilities including education, admissions, obtaining physician orders, and care coordination for Alivia Home Health clients.Cross-train to provide coverage for palliative and hospice nursing services.Perform additional duties as assigned.Required Licenses/Certifications:State of Florida RN licensureFlorida driver's license with an acceptable driving record.Education/Experience:Any combination of education and experience that would provide required skill and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:Completion of a nursing program from an accredited school of nursing, two (2) year Associate's Degree or Bachelor's Degree in Nursing.Two (2) years Registered Nurse experience within the past five years, orTwo (2) years of pediatric experience, preferably obtained while working at a pediatric hospital.Community Hospice & Palliative Care, including all of its subsidiaries and affiliated companies, is committed to complying with all applicable provisions of the Americans with Disabilities Act (ADA), as amended.
