Hospice Admissions RN: Compassionate Care Transitions
Gentiva Health Services, Inc.
Gentiva Hospice seeks an experienced Hospice Admissions Nurse to connect with patients and families at a vulnerable time. You will assess eligibility, explain hospice options with empathy, and guide the admissions process to ensure a smooth start to care. You will collaborate with physicians, case managers, and facilities to coordinate transitions and document clinical findings.
