Opportunity:

RN - Home Health Nurse

Job Details:

• 13-week contract with 40 guaranteed hours per week
• 8-hour day shifts, approximately 8:00 AM to 4:30 PM with potential flexibility in start and end times based on census
• Every 5th weekend and holiday rotation required
• On-call required 2-3 times per month
• Primary location in Wausau Home Health with potential to float to Medford, Antigo, and Wisconsin Rapids on a limited basis
• Average of 20-25 patients with 4.5 visits per day; daily travel ranges from 25-60 miles depending on territory, patient census, and staff availability
• Mileage reimbursed at the government approved rate
• Responsibilities include OASIS admissions, all time points, episode management, physician calls and coordination of care, timely EMR documentation, and clinical competencies in wound care, IV, ports, lab draws, and chronic disease management
• Charting experience with OASIS required; experience with Epic Remote Client/Remote Care preferred
• Minimum 2 years of home health experience required; prior travel experience required
• Required certifications: AHA or ARC BLS, active WI or Compact license, and valid driver's license; OASIS certification is a plus
• Associate degree or certificate required; Bachelor's degree preferred
• Scrub color: Navy blue
• Traveler must reside outside 50 miles from facility
