Nurse Practitioner for In-home Visits

Position Summary Signify Health, part of CVS Health®, seeks board-certified nurse practitioner contractors to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and selected commercial plan members. Contractors will perform medication and medical history reviews and physical evaluations in patients’ homes, using a pre‑populated patient‑specific form. They will not prescribe medication, order laboratory tests, or modify current treatment regimens. Responsibilities Conduct in‑home visits focusing on overall health, well‑being, and potential barriers to care. Review medication history, assess medical history, and perform physical examinations. Complete pre‑populated evaluation forms accurately and promptly. Adhere to Signify Health clinical protocols and documentation standards. Qualifications Board‑certified nurse practitioner. Active state licensure (required). Medicaid license or willingness to obtain one with support (preferred). Strong communication and patient‑centered care skills. Compensation & Benefits Earn up to $2,500 to $4,000 each week (average 6‑9 visits daily, base rate $80 per assessment). Per‑visit rate increases up to 135% or 160% with Market Multiplier based on county. Pay‑per‑test model for qualifying diagnostic and preventive tests. Availability incentives based on weekly work volume. Airfare, hotel, and rental car available (eligibility dependent). Malpractice coverage. Independent contractor status (1099). Schedule & Work Environment Shifts available Monday through Sunday; no nights or weekends required. Short‑term or long‑term engagement options. Pre‑scheduled visits with health plan members.
