This travel assignment provides a full range of professional nursing services to home health clients in a care-team setting, focusing on admission, evaluation, and care plan development for patients of all ages. You will work with a team of home health professionals and paraprofessionals to achieve coordinated, high-quality patient care in the designated service area.

Responsibilities
- Conduct admissions, initial assessments, and ongoing evaluations of home health patients
- Develop and adjust individualized care plans in collaboration with the patient, family, and interdisciplinary team
- Deliver professional nursing care in the home setting, including routine assessments, symptom management, and patient education
- Monitor patient status, document findings, and communicate changes to the care team to ensure coordinated care
- Educate patients and families on disease processes, medications, self-care, and safety within the home environment
- Maintain accurate records and ensure compliance with regulatory and agency policies

Requirements
- Current Vermont RN license (active)
- One year of prior clinical experience
- Ability to perform professional nursing duties in a home health setting
- Willingness to travel within the service area and work a day shift schedule
