En este rol, el Especialista en Alcance Comunitario PACE es responsable de impulsar el crecimiento del programa a través de la participación estratégica en la comunidad, el desarrollo de relaciones y la generación de referencias de alta calidad. ¿Qué es PACE? El Programa de Atención Integral para Personas Mayores (PACE) es un modelo de atención único diseñado para ayudar a las personas mayores a mantenerse seguras e independientes en sus propios hogares. Atención basada en relaciones con un impacto real: Construye conexiones a largo plazo con los participantes, apoyándolos en todas las etapas de la atención, esta es una alternativa significativa a la atención tradicional en centros o instalaciones. Enfoque preventivo y dirigido por el equipo: Colabora estrechamente con médicos, trabajadores sociales, terapeutas y otros profesionales en un modelo de atención proactivo centrado en la prevención, el bienestar y el bienestar holístico. Qué harás: Construye y mantiene relaciones sólidas con socios comunitarios, fuentes de referencia, organizaciones que atienden a los adultos mayores y proveedores de atención médica. Desarrolla y ejecuta estrategias de alcance para aumentar la conciencia del programa PACE. Genere referencias de alta calidad y guíe a los participantes y familias prospectivos a través del proceso de inscripción. Represente el programa PACE con profesionalismo, empatía y un profundo entendimiento de las necesidades de atención geriátrica. Cumpla y supere los objetivos de rendimiento vinculados a la actividad de alcance, el volumen de referencias y el crecimiento de la inscripción. Identifica poblaciones de Medicaid, Medicare, Dual Eligible, VA y Private Pay y alinea estratégicamente los servicios de Empath Life PACE dentro de sus comunidades a través de canales de referencia. Sirve como embajador de la marca en la comunidad, fortaleciendo la visibilidad y la reputación del programa. Mantenga un conocimiento profundo del mercado de atención médica de la comunidad, la competencia, las demandas del consumidor y las políticas gubernamentales que influyen en los objetivos de alcance de la agencia y el acceso para pacientes/familias.
