Position Summary

We are seeking a Full-time Physical Therapist to join our interdisciplinary team at Celebrate Senior Living in Moline. This role is ideal for a PT who enjoys working with older adults, values teamwork and wants to make a meaningful impact on resident's quality of life.

 

Job Description

• Evaluate residents and develop individualized treatment plans

• Provide skilled PT services per physician orders

• Document care accurately and timely

• Collaborate with nursing and interdisciplinary team members

• Support resident safety, mobility and functional independance

Qualifications

• Illinois Physical Therapist license (required) 

• Skilled nursing or rehab experience preferred (new grads welcome) 

• Strong communication and teamwork skills
