Minimum Requirements

• Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
• Active West Virginia Physical Therapist license required prior to start.
• Home health experience required. Experience providing patient care in home settings preferred.
• Familiarity with HCHB EMR preferred.
• BLS certification from AHA or ARC may be required.

Assignment Details

• Facility type: Home Health.
• Shift name: Days.
• Shift details: Monday–Friday, 0800–1600 (5x8).
• Weekly guaranteed hours: 40.
• Productivity expectation: 30 points per week.
• Coverage area: Kingwood, WV and surrounding two counties. Mileage reimbursed between visits from first patient to last patient.
• On-call/weekend requirement: No.
