Director of Community Outreach

The Director of Community Outreach provides strategic leadership, planning, and oversight for the hospital’s community engagement, community benefit, external partnerships, public relations, grant development support, and outreach activities. The Director serves as the primary liaison between the hospital and its affiliated Foundation, ensuring alignment of philanthropic efforts with organizational priorities and facilitating communication, collaboration, and reporting between the two entities. This position is responsible for developing and implementing initiatives that improve community health, address identified community needs, strengthen stakeholder relationships, and increase awareness of the hospital’s services and mission. The Director works collaboratively with hospital leadership, clinical departments, government agencies, educational institutions, nonprofit organizations, and community partners to advance health equity, improve access to care, and support the hospital’s strategic objectives. The Director evaluates program effectiveness through data-driven performance measures, ensures compliance with applicable community-benefit reporting requirements, and serves as a trusted ambassador for the organization throughout the community.

Essential Functions

Strategic Leadership

Community Engagement

Develop, implement, and evaluate a comprehensive community outreach and engagement strategy aligned with the hospital’s mission, strategic plan, Community Health Needs Assessment (CHNA), and identified community priorities. Identify emerging community needs, healthcare trends, and partnership opportunities, providing recommendations to executive leadership. Lead community listening efforts, stakeholder engagement activities, outreach planning, and relationship-building initiatives throughout the service area. Establish measurable goals, objectives, and performance indicators for community outreach initiatives and monitor outcomes. Translate community feedback, demographic trends, and population health data into actionable recommendations for executive leadership and governing bodies.

Community Relations

Public Representation

Serve as the primary coordinator for hospital community relations programs, community events, health fairs, educational programs, and public outreach initiatives. Build and maintain positive relationships with community organizations, healthcare partners, schools, faith-based organizations, businesses, civic groups, chambers of commerce, and local government agencies. Represent the hospital at community meetings, coalitions, public forums, conferences, and approved speaking engagements. Promote awareness, understanding, and preference for the hospital and its services among internal and external stakeholders. Serve as a visible ambassador for the organization while protecting and enhancing the hospital’s reputation and public image.

Foundation Relations

Philanthropic Support

Serve as the primary liaison between the hospital and the Foundation, ensuring alignment of Foundation activities with organizational priorities and community needs. Support Foundation leadership in the planning, development, implementation, and evaluation of fundraising activities, including annual campaigns, special events, capital campaigns, planned giving initiatives, grant opportunities, and donor engagement activities. Provide regular communication, coordination, and reporting between hospital leadership and Foundation leadership. Collaborate with Foundation representatives to identify philanthropic priorities that support patient care, community health improvement, capital investments, and strategic initiatives. Support donor stewardship and community engagement efforts that strengthen philanthropic partnerships.

Community Benefit

Population Health

Lead activities supporting the hospital’s Community Health Needs Assessment and implementation strategy processes. Coordinate community benefit planning, documentation, tracking, measurement, and reporting activities in accordance with federal, state, and organizational requirements. Promote initiatives that address health disparities, social determinants of health, access to care, and community health improvement priorities. Partner with clinical and operational leaders to develop outreach programs that improve access to preventive, primary, and specialty healthcare services. Ensure outreach efforts support health equity and culturally responsive service delivery throughout the service area.

Grant Development

Resource Stewardship

Identify, coordinate, and support grant funding opportunities that align with organizational priorities and community health needs. Collaborate with internal departments, community partners, and the Foundation to develop grant proposals and funding strategies. Assist with implementation, monitoring, compliance, and reporting requirements for grant-funded programs and initiatives. Track program outcomes and funding impacts to demonstrate community benefit and return on investment.

Government

Community Partnerships

Cultivate productive relationships with elected officials, public agencies, healthcare organizations, educational institutions, nonprofit organizations, and community leaders. Support advocacy and collaborative efforts that improve healthcare access and community well-being. Monitor relevant legislation, regulatory developments, and community initiatives affecting healthcare delivery and community health. Facilitate partnerships that advance organizational goals and improve health outcomes within the community.

Operational Management

Manage departmental operations, budgets, grants, sponsorships, contracts, vendors, events, and other resources in accordance with hospital policies and procedures. Monitor expenditures and ensure responsible stewardship of organizational resources. Develop and maintain systems for tracking outreach activities, community partnerships, program participation, outcomes, and community benefit investments. Ensure all activities comply with regulatory requirements, organizational policies, and patient privacy standards.

Leadership

Organizational Effectiveness

Inspire and engage staff, volunteers, community partners, and stakeholders through effective communication and shared purpose. Champion strategic initiatives that strengthen organizational performance and improve community health outcomes. Foster collaboration across departments to support outreach goals and organizational priorities. Promote a culture of service excellence, accountability, innovation, and continuous improvement. Perform other duties and responsibilities consistent with the position’s purpose and as assigned by leadership.

Minimum Qualifications

Bachelor’s degree in Public Health, Healthcare Administration, Business Administration, Communications, Community Development, Nonprofit Management, Social Services, or a related field; or an equivalent combination of education and directly relevant experience. Minimum of five (5) years of progressively responsible experience in community outreach, community benefit, public health, population health, nonprofit leadership, healthcare administration, government relations, foundation relations, or a related field. Minimum of three (3) years of leadership, management, supervisory, or significant project management experience. Demonstrated success developing community partnerships, leading strategic initiatives, managing programs and budgets, and measuring outcomes. Strong written, verbal, facilitation, presentation, negotiation, relationship-building, and conflict-resolution skills. Ability to analyze data and translate findings into strategic recommendations and actionable plans. Proficiency with Microsoft Office applications, virtual meeting platforms, data-tracking systems, presentation software, and reporting tools. Valid California driver’s license, acceptable driving record, and proof of automobile insurance when driving is an essential function, or ability to utilize an approved alternative means of transportation.

Preferred Qualifications

Master’s degree in Public Health (MPH), Healthcare Administration (MHA), Business Administration (MBA), Public Administration (MPA), Communications, or a related field. Experience in a rural healthcare organization, public hospital district, critical access hospital, federally qualified health center (FQHC), or community health setting. Experience supporting Community Health Needs Assessments (CHNA), community benefit reporting, grant administration, and philanthropic development activities. Knowledge of healthcare regulations, population health strategies, social determinants of health, and health equity principles. Bilingual or multilingual communication skills reflective of the communities served.
