Pharmacist - Outpatient

Pharmacist Outpatient

Day 4x10-hour shifts, weekdays only.

Onsite seeing 16 patients per day, time split between two primary care clinics.

BCACP board certification required.

PGY2 Ambulatory Care residency required.

4 years of direct patient care demonstrating medication management required.

Pharmacy license from Texas State Board of Pharmacy required.

BLS certification required.

Provide clinical leadership for pharmaceutical care in designated areas.

Develop and optimize pharmacotherapeutic regimens through collaboration.

Conduct drug utilization reviews and monitor for interactions and adverse events.

Minimize polypharmacy and create patient medication action plans.

Precept pharmacy students and residents.

Participate in multidisciplinary teams for patient care.

Communicate clinical interventions as part of the patient care team.

Offer drug information and monitoring services to practitioners.

Assist with staff development and access to continuing education.

Participate in the development of Pharmacy and Therapeutics policies.
