Arbour Hospital Discharge PlannerThe Discharge Planner functions as a member of the interdisciplinary treatment team with primary responsibilities for discharge planning. Acts as a liaison with referral sources and other departments within the Hospital.Arbour Hospital - Jamaica Plain, works diligently and compassionately to provide patients and their families with high-quality care and service excellence across a continuum of specialized behavioral health services.
