Key Responsibilities\n

• Actively act as a liaison between families and outpatient providers, ensuring effective communication and coordination of care.

• Engage with insurance companies to secure pre-certification and manage utilization effectively for the services provided.

• Participate in multidisciplinary discharge and aftercare planning meetings, offering insights into clients' unique needs and circumstances.

• Facilitate therapeutic meal support sessions, demonstrating and modeling healthy relationships with food while engaging clients nutritionally.

• Provide empathetic, safe, and affirming care tailored to each client’s distinctive identity and experiences.

• Document client interactions and treatment plans in compliance with organizational policies and maintain confidentiality in all communications.

Patient Population / Client Focus\n

The primary focus of this role is to support older adults, many of whom face complex mental health issues exacerbated by social or economic challenges. You will work closely with clients who may be experiencing anxiety, depression, grief, or adjusting to significant life changes. Compensation and Benefits\n
