Conduct comprehensive assessment of patients by determining patient needs, developing courses of action including treatment plan and outcomes. Deliver personalized rehabilitation therapy to patients in their home or care facilities. Constantly evaluate evolving patient needs and respond with plan of care adjustments. Qualifications Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern New York State License and current registration in Physical Therapy Valid driver's license will be required.
