Case Manager - Prescott Valley

Child & Family Support Services (CFSS) is accepting applications for Full‑time Case Manager in Yavapai County. The Case Manager position is ideal for a determined, experienced behavioral health technician (BHT), social worker or behavioral health professional seeking an exciting and rewarding career opportunity working with young adults and their families. This is not a supervisory position; the majority of time is spent in homes and the community rather than in an office setting. Case Manager Job Responsibilities Providing ongoing case management; coordinating behavioral health services, medical care, education, placement, finances, and socialization services to children and families with intense/complex needs. Collaborating with the family and team members to create support plans that accurately capture the strengths, needs and culture of the family. Facilitating Child and Family Team meetings; establishing an agenda, setting and maintaining a strength‑based meeting environment, staying within timeframes, giving all team participants the opportunity to contribute, holding people accountable for their commitments and assisting a team in developing a consensus for children and families. Assisting young people and families in securing needed community resources and linkages to other services; helping families identify natural and informal supports. Service delivery can be inclusive of skills training and family support. Interacting professionally with other stakeholders: Division of Child Safety (DCS), Division of Developmental Disabilities (DDD), juvenile probation, Direct Service Providers (DSP), etc. Performing other duties related to the position. Education/Experience High school diploma or GED. Must be BHT with two years of related behavioral health work experience, or a related bachelor’s degree (preferred but not required). Requirements Must be 21 years or older. Valid Arizona Driver's License with an acceptable 39‑month driving record. Reliable transportation, including proof of insurance. Candidates are required to submit a negative drug test. Obtain an Arizona DPS Level One Fingerprint Clearance Card. Complete Adult, Child, and Infant CPR and First Aid training.
