ID/DD Case Manager Western Tidewater Community Service Board Forward, Together

Western Tidewater CSB is seeking qualified Case Managers for various departments throughout our organization. ID/DD experience is preferred for this position. Responsibilities and Duties for ID/DD Case Manager: Responsible for consumer assessment, service linkage, coordination, development, and implementation of a case management ISP Assist and provide follow up instruction, education, and counseling to consumers/families with ID/DD Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits Manage consumer records according to all Licensure and Medicaid standards.
