Clinical Pharmacy Specialist - PRN (As Needed)

The Clinical Pharmacy Specialist serves as the representative of pharmacy in all aspects of the care the pharmacy department provides. They oversee review and implementation of pharmacy policies, formulary management, drug‐related education, and collaboration with other hospital departments. They also supervise clinical staff, participate in academic scholarship, and help create and execute collaborative practice opportunities at Children’s National. Functional Accountabilities Order Processing and Accreditation Readiness Interpret medication orders, oversee packaging, labeling and distribution of medications. Input orders into the pharmacy computer system when appropriate. Staff the pharmacy operation to maintain distribution skillset. Assist in performing unit inspections and verifying accreditation readiness. Review medication lists for therapeutic duplication, omissions, dosing errors, drug interactions, and allergies. Verify new medication orders to ensure appropriate drug, dose, route, and frequency. Assist nursing staff and other providers with automated dispensing machine questions. Interventions / Documentation Perform drug therapy monitoring and intervene on dosing, administration, interactions, side‑effects, and monitoring issues. Complete therapeutic drug monitoring consults and adverse reaction reports. Document interventions, drug information, and patient chart entries. Collaborate with multidisciplinary teams to ensure appropriate medication access and recommend alternative therapies. Actively participate in medication reconciliation to ensure a complete and accurate home medication list. Work collaboratively with providers to implement and communicate comprehensive medication evaluation and management, including across care settings. Staff Development and Training Assist with education, training, and supervision of pharmacy students and residents. Assist with training of new pharmacists. Provide lectures and drug reviews to professional groups and committees. Develop system changes, educational programs, and interventions that reduce prescribing, dispensing, and administration errors. Clinical Services Assure appropriate medication use throughout the organization. Identify drugs/therapies for further evaluation and formulary addition. Provide accurate and timely drug information to physicians, nurses, and other health professionals. Maintain an evidence‑based, current clinical practice. Serve as a clinical specialist resource for pharmacists, nurses and physicians, addressing and following up on patient‑care issues. Assist the department in managing drug shortages. Represent the department on key committees. Assist pharmacy operations and informatics teams with implementation and updating of formulary items. Become credentialed and help design, implement, and execute collaborative practice agreements. Quality Improvement Lead cost‑saving initiatives within the department. Lead and participate in quality improvement initiatives internally and externally. Contribute to development of order sets by devising, implementing, and testing electronic order set builds. Assist with the growth of clinical pharmacy programs throughout the organization. Participate in performance/quality improvement and patient safety activities for the service area, Pharmacy Services, and the Enterprise. Identify high‑cost, high‑use agents and participate in cost‑management initiatives. Professional Development Participate in or lead research initiatives with medical units and resident projects. Attend, present, and/or moderate at subject‑area conferences. Engage in preceptor development activities, including seminars, teaching in the Children’s preceptor development series, and completing preceptor development CEs. Contribute to the primary literature through publication and/or poster presentations. Qualifications Doctor of Pharmacy (Pharm.D.) – Required Minimum work experience: at least 2 years in pharmacy, with at least 1 year in a pediatric, specialized, academic, or clinical setting; 6 years required for candidates without a PGY2 residency. Licensed pharmacist eligible for licensure in the District of Columbia ± Maryland and Virginia (specialty dependent, 90 days).
