Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks Hours: 40 hours per week Shift: 8 hours, days Employment Type: Travel A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment. Assess referred patients that require rehabilitation services, providing direct patient care and evaluating medical condition, functional capabilities, limitations, and potential for rehabilitation. Establish and administer a treatment program with goals determined by the patient's capacity and tolerance under physician direction. Set realistic and achievable goals, document and record patient condition, and educate patients and families on appropriate physical therapy methods. Requirements Graduated from an accredited school. Current state license as a Registered Physical Therapist / PT or actively pursuing licensure. Current CPR certification. Develop a plan of care for each patient. Provide skilled physical therapy services/interventions per physician orders. Assess patient needs, plan, evaluate, and modify care to meet therapy goals. Collaborate with all disciplines to plan and evaluate team goals for each patient.
