Certified Social Worker: Acute Care & Discharge Planning

ProLink is seeking a Certified Social Worker to provide psychosocial support to patients and families in hospital or long-term care settings in Duluth, GA. You will assess needs, develop support plans, and help with discharge planning.
You will document interventions in EHR systems such as Epic, collaborate with healthcare teams, advocate for patients, and coordinate community resources to ensure holistic care.
