RN: Case Manager

Job Description: Join a travel contract as a Registered Nurse Case Manager in Atlanta, GA, supporting day shift operations Monday through Friday with a focused, plan-driven workflow. In this role, you’ll assess and evaluate discharge needs, coordinate services to the appropriate level of care, and ensure the treatment plan is initiated, evaluated, and re-evaluated daily. You’ll also track progress and identify barriers to discharge while monitoring length of stay—keeping momentum where it matters most. This assignment emphasizes hands‑on, regulatory‑aware case management, including adherence to Medicare guidelines related to patient choice, IMM notices, and MOONS. You’ll work within facility expectations for consistent daily follow‑through, with no call and no weekend shifts. To be considered, you’ll need a minimum of 3 years of relevant experience, and first‑time travelers are not accepted.
