RN Health Coach Hybrid Evernorth

Heres how you will help our patients:

• Identify customer health education needs through targeted health assessment activities. Refer customer to other Cigna Evernorth clinical programs or client specific programs as clinically appropriate.
• Collaborate with customers to establish health improvement plans set personalized evidence-based goals and support customers in achieving those goals. Empower customers to become an active participant in their own health outcomes.
• Assist Customer in overcoming barriers to better health
• Lead and support a variety of Health and wellness promotional activities such as group coaching wellness challenges and Health related seminars.
• Utilize biometric values and motivational interviewing techniques to collaborate with customers to drive to improve clinical outcomes.
• Collaborate with our matrix partners to support the overall health and wellness strategy your aligned client.
• Understand and own a variety of clinical targets and outcome measurements.
• Perform Stress anxiety and depression screening
• Develop action plans that drive clinical value for the customers and clients.
• Additional responsibilities in alignment with the scope of your certification or licensure
• Provide support in training or mentorship of new employees as needed

Expected Credentials to best serve our patients:

• Strong Clinical skills with at least 1 or more years of experience in health coaching health education and health promotion
• RN required with active unrestricted license in respective state and ability to maintain
• Compact license or additional state licenses may be required with ability to maintain
• Current ACLS/BLS/CPR/AED Certification
• Minimum of an Associates Degree in Nursing; Bachelors Degree strongly preferred
• High-speed internet and quiet office space in dedicated home office
• High energy level with dynamic presentation skills is required.
• Positive role model in demonstrating healthy behaviors
• Passion for health improvement
• Ability to work independently
• Customer-centric focus
• Ability to proactively collaborate professionally with the client and other matrix partners.
• Proven administrative abilities with strong computer and software application skills.
• May support early or later hours depending on customer needs

Bonus points for :

• Behavioral Health experience including stress reduction
• Masters degree in psychology or related field is preferred
• Chronic Disease Management experience
• NBHWC (National Board-Certified Health & Wellness Coach)
• Motivational interview training/experience

RN Licensure note : Ideal candidate will have a compact (multi-state) RN license and be licensed in the state of New York.

Language : Bilingual in Spanish

Location: Hybrid; 2 days/week remote 3 days/week onsite at Leviton plant in Morganton NC
