Location: Westbury, NY | Schedule: 35 hours per week | Status: Non-Exempt |Salary:20.61/Hour

The Opportunity

The Young Adult Recovery Care Coordinator provides comprehensive care coordination and service navigation for young adults ages 18-24 residing in recovery residences across Long Island. This role addresses Social Determinants of Health (SDoH) needs, connects participants to essential clinical, social, and vocational resources, and ensures continuity of care across systems. Working closely with the Young Adult Residences Supervisor and Recovery Coaches, the Care Coordinator supports participants in achieving stability, wellness, and sustained recovery.

Responsibilities

• Conduct comprehensive, strengths-based assessments, including identification of Social Determinants of Health (SDoH) needs, and collaborate with participants to develop and update individualized service plans.

• Coordinate care and service navigation for young adults in recovery residences, ensuring timely access to behavioral health, medical, vocational, educational, and social supports.

• Facilitate referrals and provide warm handoffs, while conducting ongoing follow-up with participants and providers to ensure successful connection to services and reduce barriers to care.

• Support participants in securing and maintaining benefits, entitlements, insurance coverage, and other financial resources critical to stability and recovery.

• Serve as a central point of contact for external providers, treatment programs, and community partners to ensure continuity of care, especially during transitions between systems (e.g., treatment to housing).

• Monitor participant progress, track outcomes, and proactively identify service gaps or emerging needs; collaborate with the Supervisor and Recovery Coaches to implement solutions.

• Coordinate and track program-related financial stipends and ensure all supporting documentation meets program and funder requirements.

• Maintain documentation of all care coordination activities, referrals, follow-ups, and outcomes in compliance with agency standards and reporting expectations.

• • Qualifications:

• High School Diploma or equivalent required; Associate's or Bachelor's degree in Human Services, Social Work, or related field preferred.

• Experience working with young adults, individuals in recovery, or housing-related programs preferred.

• Knowledge of community resources, entitlement/benefits systems, and behavioral health service networks required.

Proficient computer skills required with Microsoft Office required.

• Excellent verbal and written communication skills required.

• Valid and clean driver's license required.
