Position Overview

We are seeking an experienced Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs. This role combines community-based field visits with remote care coordination, offering flexibility while making a meaningful impact on members' health and quality of life.

Responsibilities

• Conduct in-home assessments to evaluate members' clinical, functional, and social needs
• Develop and manage individualized care plans in collaboration with physicians and interdisciplinary teams
• Coordinate care across home care, hospitals, rehabilitation facilities, and long-term care settings
• Complete approximately 6 home visits per week within the Bronx territory
• Monitor member progress and adjust care plans to support optimal outcomes
• Support safe discharge planning and help reduce hospital readmissions
• Communicate effectively with members, families, caregivers, and healthcare providers
• Maintain accurate and timely clinical documentation in accordance with applicable guidelines
• Advocate for members and connect them with appropriate healthcare and community resources

Qualifications

• Active, unrestricted New York RN license
• Associate's degree in nursing required; BSN preferred
• Minimum 3 years of RN experience
• Experience in MLTC, home care, geriatrics, or case management preferred
• Strong communication, organizational, assessment, and critical-thinking skills
• Ability to work independently in a hybrid environment
• Bilingual skills are a plus

• Field Schedule: Approximately 6+ home visits completed in a week
• Training: Onsite at Queens office
• Salary: $100,000–$130,000 per year
