Title
RN - Discharge Planning / Care Coordination Schedule
Monday - Friday (block schedule) Requirements
- RN license required (Georgia or Compact license)
- Minimum 3 years of RN experience
- BLS certification
- Travel experience required; not accepting first-time travelers
- Community hospital experience preferred Roles & Responsibilities
- Assess patient needs and initiate and evaluate treatment plans.
- Drive discharge planning and coordinate services to the appropriate level of care.
- Monitor length of stay and identify barriers to discharge.
- Ensure compliance with Medicare guidelines and other regulatory standards.
- Conduct concurrent reviews and utilization reviews as necessary. Compliance / Notes
- Full compliance required to start on time; non-compliance may delay start by approximately 2 weeks.
- All compliance documents due by Tuesday prior to start.
- Orientation and modules are non-billable; hours vary based on contract length.
