Home Health Speech Language Pathologist As a Home Health Speech Language Pathologist, you will: Evaluate, direct, and provide speech/language pathology services to patients in the home or facility Participate in the development and periodic review of the Plan of Treatment and Plan of Care Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders and conditions Administer and interpret diagnostic tests and applications of therapeutic treatments, including audio logic screening Observe, record, and report changes in the patient’s condition and response to treatment to supervisor and/or the physician Provide instruction and training to patients in the use of alternative communication systems when appropriate Provide counsel and instruction to patients, families, and healthcare staff Maintain and submit documentation as required by the company and/or facility, including written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports in accordance with company policy Participate in care coordination activities and discharge planning Maintain the highest standards of professional conduct concerning confidential information, sharing data only when the recipient’s right to access is established and in the best interests of the patient Required Qualifications Minimum six months experience as a speech therapist / speech language pathologist Current and unrestricted license Current CPR certification Valid driver’s license, auto insurance, and reliable transportation Preferred Qualifications Home Health experience Pay Per Visit/Unit Range $54.00 - $76.00 per visit/unit TB Statement This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. Driving Statement This role is part of Humana’s driver safety program and therefore requires an individual to have a valid state driver’s license and to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, or whichever is higher. Pay Range $85,400 - $117,500 per year (full time 40 hours per week).
