Registered Nurse - Home Health

Job Description

Job Summary:
Provides comprehensive home nursing care coordination and delivery in accordance with physicians' orders, agency policies, and procedures. Oversees and supervises home health aides and licensed practical or vocational nurses to ensure quality patient care.

Responsibilities:

• Coordinate and administer patient care visits, including start of care, wound care, and follow-up visits.
• Supervise and evaluate the performance of Home Health Aides, LPNs, and VNs.
• Document patient interactions accurately within electronic health record systems such as HomeCare HomeBase (HCHB) and PointCare.
• Travel up to 75 miles from the home clinic to patient residences, managing 5-8 patients daily.
• Adhere to scheduling requirements, including weekend rotations and on-call duties every 3-4 weeks.
• Ensure timely completion of all documentation within 24 hours of each visit.

Qualifications:

• Hold an active RN license in the state of practice.
• Possess a minimum of 1 year full-time experience in adult home health or hospice care.
• Proficient in Oasis documentation and recent use within the last 6 months.
• Have a reliable vehicle, valid driver's license, and sufficient auto insurance.
• Demonstrate excellent communication and clinical skills essential for in-home patient care.
• Able to travel 50-75 miles weekly and manage a caseload of 5-8 patients per day.
