Summary of Physical Therapist Job Description The Physical Therapist will be responsible for providing outpatient physical therapy services for Odom Rehab in an ethical and professional manner, in accordance with state guidelines. The employee may perform other duties as are customarily performed by a similar position and other duties as assigned by Odom Rehab from time to time. Essential Functions Provide physical therapy services Evaluate and treat patients in accordance with state guidelines, identifying root causes of dysfunction that translate to impairments in daily function Establish appropriate Plan of Care (POC) for patients outlining the path to achieve goals; reassess periodically to keep goals relevant Oversee treatment provided by the PTA based on established POC Deliver treatment including manual therapy, therapeutic exercises, training in movement and body mechanics, balance development, and pain management to reach goals Educate patients on self-management and educate family members on how to assist Update POC as necessary for patient care Assess appropriate adaptive equipment (e.g., canes, walkers, orthotics) Develop discharge plans for patients upon completion of therapy Document patient treatment daily according to practice guidelines Submit charges daily for Physical Therapy services provided on that date Communicate regularly with physician and medical staff regarding patient status or changes Assist in developing and updating ODOM REHAB treatment protocols as needed Actively participate in ODOM REHAB staff meetings Track and complete Continuing Education Credits and renewal steps to maintain licensure Maintain an active Physical Therapy license Core Responsibilities For All OHW Employees Actively promote and uphold OHW’s Mission: to treat the whole person with the knowledge that we are all created to move At Odom Rehab, strive to empower the community toward independent and healthy living Demonstrate OHW Core Values: Servant Heart, Exceptional Quality, People First, Continuous Improvement, Empathy & Integrity Compliance and Policies Comply with OHW policies and procedures and applicable regulations, including HIPAA. Qualifications Certifications Graduation from an accredited Physical Therapy program with a degree in Physical Therapy and current MN State Physical Therapy licensure One (1) year of experience as a Physical Therapist (preferred) Recent/previous work experience in a primary health care setting (preferred) Additional Qualifications Ability to follow written and oral directions Computer skills required Demonstrates personal and professional accountability and responsibility Self-directed and able to perform duties independently Ability to problem-solve and apply critical thinking Maintain confidentiality For mandatory administrative drivers: valid Minnesota driver’s license, acceptable driving record, and access to a reliable vehicle Maintain personal vehicle auto insurance coverage Working Environment and Physical Requirements Work Rating: Medium work. Medium work involves lifting up to 50 pounds at a time with frequent lifting or carrying of objects up to 25 pounds; may require sedentary and light work as well. Driving Requirements: This position is designated as Mandatory Driving: Administrative. OHW conducts background checks including driving record review for employees who must drive in their position.
