This 13-week travel social work assignment is ideal for an experienced hospital social worker who excels in psychosocial support, safety planning, discharge planning, and patient placement coordination. Located in South San Francisco near the heart of the Bay Area, this on-site opportunity offers the appeal of a well-connected community with convenient access to local dining, shopping, and nearby city attractions while supporting patients as part of an interdisciplinary team in an Epic EMR environment. Weekly Estimated Pay: $2,030-$2,190 Start Date: 8/3/2026 Shift Duration: 10 Hour Days Setting: On-site hospital social work / medical social work EMR: Epic Job Requirements Master of Social Work (MSW) required At least 1 year of hospital-based social work experience required Experience with psychosocial assessments, safety planning, and crisis support Ability to collaborate with interdisciplinary healthcare teams and community resources Experience supporting discharge planning, care coordination, and placement coordination preferred Strong communication, documentation, patient advocacy, and case management skills Responsibilities Provide psychosocial support and clinical social work assessment for patients in a hospital setting Develop and implement safety plans based on patient needs and care goals Collaborate with physicians, nurses, case managers, and care teams on discharge planning and next-step placement Assist with detox placement coordination and related resource planning Document patient interactions, care coordination, and case notes accurately in Epic Advocate for patients and families while connecting them to community, behavioral health, and clinical resources 401K with Matching, Healthcare, Dental and Vision We are an equal opportunity employer and value diversity across our organization.
