The therapist may work evenings, weekends, weekdays, or a set recurring schedule depending on mutual agreement and client demand.\n\nThe provider may maintain a part-time or ongoing caseload and is expected to manage scheduling, attendance, documentation, and communication in accordance with agency procedures.\n\nCOMPENSATION STRUCTURE: This is a fee-for-service role. The therapist will provide individual, couples, family, and group therapy services to children, adolescents, adults, couples, and families experiencing emotional, behavioral, relational, and mental health concerns.\n\nThe Licensed Marriage and Family Therapist will be responsible for completing clinical assessments, treatment plans, therapy sessions, progress notes, family-based interventions, crisis support within outpatient scope, and care coordination. The role may include maintaining an ongoing caseload and supporting clients through evidence-based and family-systems-informed treatment.\n\nThis position is ideal for a clinician who is comfortable working independently, managing a flexible caseload, and delivering high-quality outpatient therapy services in a community mental health setting.\n\nSCHEDULED DUTIES AND RESPONSIBILITIES\nClinical Assessment and Intake Services\nThe Licensed Marriage and Family Therapist may be responsible for:\n\nConducting initial clinical assessments and biopsychosocial evaluations.\nGathering client, family, relationship, behavioral, trauma, developmental, and mental health history.\nIdentifying presenting problems, family dynamics, relationship stressors, symptoms, strengths, risks, and treatment needs.\nCompleting diagnostic impressions within scope of practice.\nAssessing for safety concerns, including suicidal ideation, homicidal ideation, abuse, neglect, domestic violence, substance use concerns, and other risk factors.\nMaking appropriate recommendations for therapy, psychiatric evaluation, case management, higher level of care, community resources, or additional clinical services.\nUpdating assessments as clinically needed.\n\nIndividual Therapy\nThe therapist may provide individual therapy to clients presenting with concerns such as:\n\nDepression.\nAnxiety.\nTrauma-related symptoms.\nGrief and loss.\nBehavioral concerns.\nStress management.\nAdjustment difficulties.\nMood instability.\nRelationship problems.\nSelf-esteem issues.\nParenting stress.\nFamily conflict.\nSchool, work, or social difficulties.\nEmotional regulation concerns.\nLife transitions.\nAnger management.\nCo-occurring family and relational stressors.\n\nCouples and Relationship Therapy\nThe Licensed Marriage and Family Therapist may provide therapy services for couples and partners, including support with:\n\nCommunication stabilization.\n\nFamily Therapy\nThe therapist will use family-systems-informed approaches to help families address:\n\nParent-child conflict.\nSibling conflict.\nBehavioral problems in children or adolescents.\nCommunication breakdowns.\nFamily transitions.\nCo-parenting concerns.\nBlended family adjustment.\nTrauma impact on family functioning.\nCaregiver stress.\nSchool and home behavioral issues.\nFamily roles, boundaries, and patterns.\nSupport for families managing mental health diagnoses.\nReunification or family stabilization goals, when clinically appropriate.\n\nGroup Therapy, If Applicable\nThe therapist may facilitate or co-facilitate groups based on agency needs, including:\n\nParenting groups.\nAnger management groups.\nCoping skills groups.\nFamily support groups.\nRelationship skills groups.\nTrauma-informed groups.\nEmotional regulation groups.\nAdolescent or adult therapy groups.\nPsychoeducation groups.\n\nTreatment Planning\nThe therapist will be responsible for:\n\nDeveloping individualized treatment plans.\nEstablishing measurable goals and objectives.\nReviewing and updating treatment plans according to agency and payor requirements.\nEnsuring treatment plans reflect client needs, diagnosis, strengths, barriers, and clinical interventions.\nIncorporating family, caregiver, or partner involvement when appropriate and authorized.\nDocumenting progress toward treatment goals.\n\nClinical Documentation\nThe therapist must complete accurate, timely, and compliant documentation, including:\n\nIntake assessments.\nDiagnostic assessments.\nTreatment plans.\nProgress notes.\nFamily therapy notes.\nCouples therapy notes.\nGroup therapy notes, if applicable.\nRisk assessments.\nCrisis intervention documentation.\nDischarge summaries.\nCoordination of care notes.\nAuthorization-related documentation, if required.\n\nAny other required documentation under agency, licensing, Medicaid, commercial insurance, or regulatory standards.\n\nCrisis Support Within Outpatient Scope\nThe therapist may provide appropriate outpatient-level crisis support, including:\n\nAssessing client safety during sessions.\nCreating and updating safety plans.\nConsulting with clinical leadership when risk concerns arise.\nMaking mandated reports when required.\nReferring to emergency services, crisis response, mobile crisis, inpatient care, or higher levels of care when clinically necessary.\nDocumenting all risk-related interventions and referrals.\n\nCare Coordination and Collaboration\nThe therapist may coordinate with:\n\nClients and families.\nParents, guardians, spouses, or partners when authorized.\nPsychiatrists and psychiatric nurse practitioners.\nPrimary care providers.\nSchools.\nCase managers.\nSocial workers.\nHospitals or crisis teams.\nChild welfare or court-related systems when authorized or legally required.\nInternal administrative and clinical leadership.\nOther outpatient mental health providers.\n\nServices the Licensed Marriage and Family Therapist May Provide\nThe therapist may provide services within their license, training, and scope of practice, including but not Freedom Healthcare LLC dba Community Wellness Outpatient Mental Health Center. The therapist may coordinate with the clinical director, administrative staff, credentialing staff, billing staff, and other clinical team members as needed.\n\nCompliance Expectations\nThe therapist must comply with:\n\nApplicable state licensing board requirements.\nHIPAA and confidentiality rules.\nProfessional ethics for marriage and family therapists.\nAgency documentation policies.\nMedicaid and commercial insurance documentation requirements, if applicable.\nMandated reporting laws.\nInformed consent requirements.\nClient rights and privacy protections.\nScope-of-practice requirements.\nTimely completion of documentation.\nProfessional conduct and boundaries.\n\nPHYSICAL DEMANDS: \n\nProlonged periods of sitting, using a computer, reviewing clinical documentation, preparing psychological evaluation reports, and completing electronic health record entries.\nFrequent use of standard office and clinical equipment, including computers, telephones, testing materials, scoring tools, and secure documentation systems.\nAbility to communicate verbally and in writing with clients, families, clinical team members, administrative staff, and supervisors.\nMay require occasional standing, walking, reaching, bending, or carrying testing materials, files, or office supplies.\nMust be able to maintain attention to detail during clinical interviews, psychological testing, scoring, report writing, and feedback sessions.\n\nWORK CONDITIONS\n\nOutpatient mental health center setting with services provided through Community Wellness Outpatient Mental Health Center, operated by Freedom Healthcare LLC, with staffing placement coordinated through Enterprise Management Solutions LLC.\nWork may include in-person clinical services, psychological testing appointments, feedback sessions, documentation, report writing, care coordination, and administrative communication.\nThe provider may work independently on a fee-for-service basis while coordinating with clinical leadership, supervisors, administrative staff, credentialing staff, billing staff, and other clinical team members.\nWork may involve interaction with clients experiencing mental health symptoms, emotional distress, behavioral concerns, trauma histories, family concerns, or complex clinical presentations.\nThe role requires compliance with HIPAA, confidentiality standards, agency documentation policies, licensing board requirements, informed consent requirements, and professional ethical standards.\nScheduling may vary based on provider availability, client need, testing referrals, office workflow, and agency requirements.\n\nCOMPETENCIES AND SKILLS\nThe ideal provider should demonstrate:\n\nStrong clinical judgment.\nExcellent communication skills.\nAbility to engage individuals, couples, and families.\nAbility to manage conflict in family or couples sessions.\nStrong documentation and time-management skills.\nKnowledge of diagnosis and treatment planning.\nAbility to work with diverse populations.\nCultural humility and sensitivity.\nTrauma-informed approach to care.\nEthical decision-making.\nProfessional boundaries.\nReliability and responsiveness.\nAbility to manage a caseload independently.\nAbility to follow agency policies and documentation timelines.\nUnderstanding of HIPAA, confidentiality, informed consent, mandated reporting, and professional ethics.\n\nLEVEL OF EDUCATION / TRAINING / QUALIFICATIONS\nMinimum Qualifications\nApplicants must have:\n\nActive and unrestricted license as a Marriage and Family Therapist in the applicable state of practice.\nMaster’s degree or higher in marriage and family therapy, counseling, psychology, social work, or a related behavioral health field accepted for LMFT licensure.\nAbility to provide outpatient mental health therapy within scope of licensure.\nExperience working with individuals, couples, families, children, adolescents, or adults.\nKnowledge of family systems, relational therapy, trauma-informed care, and behavioral health treatment planning.\nAbility to complete timely and compliant clinical documentation.\nAbility to work independently on a fee-for-service basis.\nProfessional liability insurance, if working as an independent contractor.\nAbility to meet credentialing, onboarding, background check, and payer enrollment requirements, if applicable.\n\nPreferred Qualifications\nPreferred candidates will have experience with:\n\nOutpatient mental health centers.\nCommunity mental health settings.\nMedicaid and commercial insurance documentation standards.\nChildren, adolescents, adults, couples, and families.\nTrauma-informed therapy.\nFamily systems therapy.\nParenting
