Pay Rate: $2,600 To $2,800/Week


Contract: 13 weeks


Schedule: 8-hour shifts, 8:00 AM - 4:30 PM

About the Role


We are seeking a Care Coordinator RN for a 13-week contract opportunity in Chico, California. This role is responsible for coordinating patient care across the continuum, collaborating with multidisciplinary teams, optimizing resource utilization, and ensuring high-quality, cost-effective care throughout each patient's episode of treatment.


Responsibilities
• Coordinate patient care plans in collaboration with physicians, patients, families, payors, and the interdisciplinary care team
• Perform comprehensive patient assessments, care planning, intervention, and evaluation using the nursing process
• Facilitate utilization management, discharge planning, observation reviews, readmission prevention, and resource coordination
• Advocate for patients and families while ensuring effective communication throughout the episode of care
• Document patient care activities accurately and support care coordination using EPIC EMR


Requirements
• Active California RN License required
• Minimum 1 year of recent Case Management experience in an acute care setting required
• Current BLS/BCLS Certification required
• Experience with EPIC EMR preferred
