About the Position

• Deliver high-quality nursing care to patients in an acute rehab setting.
• Assess, plan, implement, and evaluate individualized patient care plans.
• Monitor neurological status and recognize changes in patient condition.
• Administer medications and treatments per provider orders.
• Assist with patient mobility and transfers using equipment such as Hoyer lifts and sit-to-stand lifts.
• Collaborate with physical therapy, occupational therapy, and other disciplines to promote optimal recovery.
• Document all patient care and progress accurately in Epic (preferred).
• Educate patients and families on care plans, safety, and discharge goals.
• Maintain infection control and safety standards in compliance with hospital policies.

Requirements

• License: Active NY RN license (required)
• Experience: Minimum 1 year of recent acute rehab or hospital experience
• Certification: BLS (required)
• Charting System: Epic (preferred)
• Patient Ratio: Days 1:5 | Nights 1:12
• Float Requirement: Yes – Medical/Surgical units
• Weekend Coverage: Every other weekend
