Remote NBC-HWC Health Coach for Chronic Care

9amHealth seeks a health coach (1099 independent contractor) to support members with obesity, diabetes, hypertension, and dyslipidemia. You will provide motivational coaching, personalize sessions using data, and collaborate with RDs to reinforce nutrition and lifestyle plans.

Responsibilities include documentation in EMRs, scheduling, and delivering care remotely from a private workspace, anywhere in the U.S., while upholding NBC-HWC standards.
